Join us for the next Cause Mapping Root Cause Analysis Public Workshop in HOUSTON on April 30 – May 2.
Fifty years ago this week, an explosion on the Apollo 13 spacecraft shifted its mission from a moon landing to a rescue. Made famous in a movie starring Tom Hanks as Commander Jim Lovell, the three astronauts aboard the damaged spacecraft and the NASA team on Earth had to employ creative problem-solving strategies… and quickly. “You sit back and find out what you have to work with and who can help you do the work,” Lovell said. Although the situation was dire, an impressive feat of ingenuity, teamwork and leadership led to the crew’s safe return to Earth on April 17, 1970 in what NASA later called, “a successful failure.” This 45-minute case study webinar will use firsthand insights from former NASA employees to explore how understanding the cause-and-effect relationships within an incident can reveal additional effective solution opportunities and work process improvements.
