Jon Bernardi

Cause Mapping RCA Investigator & Instructor

Jon has a Bachelor of Science in Chemical Engineering from the University of Illinois, in Urbana/Champaign, and an MBA from Bellarmine University in Louisville, Kentucky. Jon worked for the Lubrizol organization for 36 years in process engineering, project engineering, manufacturing management, quality/safety systems, and manufacturing process safety.

Jon began using the Cause Mapping method in 2002 and has since been involved in leading, training, and investigating incidents. He delivers workshops for analyzing, documenting, communicating, and solving problems effectively and provides consulting services for incident investigation using the Cause Mapping methodology.
