Mark Galley - Cause Mapping RCA Investigator & Instructor
Mark has a Bachelor of Science in Mechanical Engineering from the University of Colorado in Boulder and obtained his certification as a Reliability Engineer in 1993 through the American Society for Quality. He is a regular presenter at national conferences and is a member of the National Speakers Assocation.
Mark’s practical experience in root cause analysis and work process reliability was developed during his time at the Dow Chemical Company where he worked for almost 9 years. He has been facilitating incident investigations and teaching workshops on root cause analysis for 15 years. Mark founded ThinkReliability in Houston in 2000. His work spans several industries including manufacturing, power generation, aviation, refining, telecommunications, healthcare, information technology, aerospace, marine and transportation.

Aaron Cross - Cause Mapping RCA Investigator & Instructor
After earning his degree in Industrial Engineering from Kansas State University, Aaron worked at Sprint telecommunications for four years where he led company wide process improvement initiatives including design and development of new and existing processes.
In 2004, Aaron began working exclusively on facilitating group problem solving and incident investigation for ThinkReliability. Currently, his work involves traveling worldwide instructing workshops on root cause analysis and facilitating problem investigations. His work spans several industries including petrochemical, power, aviation, refining, telecommunications, healthcare, information technology, aerospace, marine and transportation.
Aaron resides in Overland Park, KS.

Angela Griffith – Cause Mapping RCA Investigator & Customer Solutions
Angela has a Master of Engineering Management degree from Old Dominion University in Norfolk, Virginia and a Bachelor’s Degree in Nuclear Engineering from Purdue University in West Lafayette, Indiana. She is a licensed Professional Engineer (Mechanical Engineering).
Prior to joining ThinkReliability, Angela worked for the Naval Nuclear Propulsion Program providing fleet support for nineteen operating nuclear reactors on NIMITZ Class aircraft carriers, coordinating emergency management communications planning for sea- and shore-based facilities, and overseeing the manufacturing of the nuclear reactors for VIRGINIA Class submarines.
Angela’s work at ThinkReliability spans several industries including manufacturing, aviation, supply-chain/distribution, transportation, nuclear and healthcare. She leads comprehensive investigations by collecting and organizing all relative information into a coherent record of the issue. Angela can thoroughly dissect both problems and processes to help the client meet their overall goals.

Loyd Hamilton - Cause Mapping RCA Investigator & Instructor
Loyd has a Bachelor of Science in Mechanical Engineering from Lamar University. Loyd has 20+ years of experience as an Engineer, Manager, Instructor and Investigator. Loyd was also a journeyman pipefitter. He has managed teams for maintenance and capital projects.
He has facilitated teams through their investigations in a variety of industries including oil refining, oil production, chemical plants, power generation, food processing, healthcare and marine.

Ely Wilson - Cause Mapping RCA Investigator & Instructor
Ely has a Master of Engineering degree in Electrical Engineering and Computer Science from the Massachusetts Institute of Technology and an MBA from the Naval Postgraduate School in Monterey, California. Ely’s experience includes the development of an autonomous robotic vehicle and 11 years directing the design, construction, testing, operation, and maintenance of propulsion systems for U.S. Navy submarines.
Ely leads problem investigations and teaches root cause analysis workshops for companies in such industries as manufacturing, aviation, refining, information technology, aerospace, marine, and transportation.
Ely has four children and lives near Colorado Springs, Colorado.

Aimee Galley - Operations & Finance
After earning a Bachelor of Science in Mechanical Engineering from the University of Washington in Seattle, Aimee relocated to Texas to begin work at the Dow Chemical Company in 1996. She started as a plant engineer within Environmental Operations then moved into Supply Chain and Production Planning. Aimee also holds a Masters in Business Administration from the University of Houston. Aimee and Mark have been married since 1999 and started ThinkReliability together in 2000. She is currently the Operations and Financial Manager at ThinkReliability, in addition to serving as lead shuttle driver for all basketball and soccer activities for their two daughters.

Sarah Barton - Marketing and Events, Customer Solutions
Sarah has a wide range of experience in Event Planning & Marketing – from specialized client events to week-long conference for over 2,500 people, and also creating the marketing communications for these events. Prior to joining ThinkReliability, Sarah worked as the Conference Manager for the Human Resources Institute of Alberta and the Canadian Society of Petroleum Geologists. Sarah has worked with people at all levels and knows that her job is done right when the attendee has a great experience. She has an Associate in Business Administration - Marketing and an Event Management Certificate through Mount Royal University.
Sarah is originally from Calgary, Alberta, Canada and is enjoying all the new adventures in Texas with her husband and two small children.

Jon Bernardi - Cause Mapping RCA Investigator & Instructor
Jon has a bachelor of science degree in Chemical Engineering from the University of Illinois, in Urbana/Champaign and an MBA from Bellarmine University in Louisville, Ky. Jon worked for the Lubrizol organization for 36 years in process engineering, project engineering, manufacturing management, quality/safety systems and, most recently, as the process safety manager with global responsibility for manufacturing process safety. Jon began using the Cause Mapping method in 2001 and has since been involved in leading, training and investigating incidents related to process, safety and work flow.
Jon is located in San Marcos California. An avid car enthusiast, he can sometimes be found working on Cause Maps for his 1967 Mercedes 250 SL.

Renata Voorhees- Cause Mapping RCA Investigator &Customer Solutions
After graduating with a Bachelor of Science degree in Mechanical Engineering from Colorado State, Renata moved to Houston, Texas to work as a pressure vessel engineering at Fluor. She was involved in maintaining and implementing new strategic Mechanical (static) suppliers throughout the supply chain. She is currently pursuing her Master of Science in Industrial Engineering with an emphasis in Capital Projects & the Supply Chain. At ThinkReliability, Renata is a Cause Mapping Investigator and Project Coordinator.
Renata is based out of ThinkReliability's office in Pearland, TX.

Kim Smiley- Cause Mapping RCA Investigator
Kim has a Master of Science in Engineering Science (Mechanical Engineering) from the Naval Postgraduate School and a Bachelor’s Degree in Mechanical Engineering from Rensselaer Polytechnic Institute in Troy, New York. She is a licensed Professional Engineer (Mechanical Engineering).
Prior to joining Think Reliability, Kim worked for the Naval Nuclear Propulsion Program in the Surface Ships fluid system division. She provided fleet support for the Nimitz Class carriers and worked on development of the propulsion plant design of the next generation air craft carrier.
Kim lives with her family near Washington D.C.

Kristin Hort - Cause Mapping RCA Investigator
Kristin has a Master of Science in Product Development Engineering from the University of Southern California and a Bachelor of Degree in Chemical Engineering from Rensselaer Polytechnic Institute in Troy, New York. Prior to joining ThinkReliability, Kristin served as an assistant professor of aerospace studies at the Air Force Reserve Officers Training Corps detachment at the Massachusetts Institute of Technology, training college studies in the leadership and management skills needed to become a junior Air Force officer. For the first four years of her military career she worked as a developmental engineer on the Airborne Warning and Control System (AWACS) platform at Hanscom AFB, MA.
Kristin lives with her family near Boston, Massachusetts.
